This is a Person Centred Planning tool that can be used with individuals and organisations.
It offers the opportunity to extend on Map and put into place a plan of action.
Path works well when an individual has a group of people around them who are committed to making things happen. A process facilitator and graphic recorder who graphically presents the information are required for effective planning with path.
