Where an individual needs extra support we provide a period of one to one individual support to connect people to their community. Through the person's interests, strengths and capacities identified through person centred planning this process leads to involvement in employment, volunteering, learning, social and recreational activities. This encourages increased activity in the community and more opportunity to build social networks. The outreach worker offers encouragement and support to the individual and their family as they embark on new experiences.
